These Plan Terms describe the features, limits, fees, and mechanics of Onfire Health membership plans. They are incorporated by reference into your Onfire Health Membership Agreement and presented to you at the time of enrollment. In the event of any conflict between these Plan Terms and the Membership Agreement, the Membership Agreement controls unless these Plan Terms expressly provide otherwise.
1. Fixed Plan — Ember
Subscribe to a specific health program bundle. No membership fee. Pay monthly.

Ember is a Fixed Plan designed for members subscribing to a specific health program bundle offered by an Onfire Health Partner Clinician or Practitioner. The member selects a health program bundle at enrollment, and Onfire Health pays the Partner Clinic from the segregated member funds account on the member's behalf. The member makes equal monthly subscription payments over the plan term via auto-debit from their Linked Bank Account. Ember is ideal for first-time members and those seeking to subscribe to a single defined program without an annual membership commitment.
Key characteristics of the Ember plan:
- –No annual Membership Fee.
- –Subscription Limit is set to the cost of the selected health program bundle at enrollment; it does not roll over or accumulate.
- –Monthly subscription payments are made by equal auto-debit from the member's Linked Bank Account.
- –Plan term is determined by the health program bundle selected, up to a maximum of 12 months.
- –Maximum Subscription Limit is $30,000 per enrollment (subject to change as published in updated Plan Terms).
- –Members may enroll in a new Ember plan upon completion of a prior plan.
- –Ember does not include Onfire Points, Other Linked Account access, or rollover benefits.
- –Member contributions are non-fungible and may only be applied to the specific health program bundle selected at enrollment.

Example: A member enrolls in a 6-month functional medicine program bundle at a cost of $4,200. Onfire Health pays $4,200 to the Partner Clinic from the segregated member funds account. The member's Subscription Limit is set to $4,200 and they make monthly subscription payments of $700 for 6 months via auto-debit from their Linked Bank Account. Upon completion, the member may enroll in a new Ember plan.
2. General Plan Terms
2.1 Enrollment
Plan selection and Subscription Limit configuration occur at the time of enrollment. The specific plan, Subscription Limit, funding schedule, plan term, and monthly subscription payment amount are disclosed to the member at the time of enrollment and constitute part of the member's agreement with Onfire Health under the Membership Agreement. The Membership Account will not be activated until Onfire Health has completed Know Your Customer (KYC) identity verification for the member.
2.2 Auto-Debit Requirement
All plans require a Linked Bank Account and authorization for auto-debit (Member Debit). This is a condition of enrollment. For Ember plans, auto-debit covers monthly subscription payments for the duration of the plan term. For Flex Plans (when available), auto-debit covers optional Membership Fees and periodic Membership Account funding contributions.
2.3 Non-Fungible Contributions; Restricted Use
Member contributions to the Membership Account are non-fungible and may only be applied toward eligible health program bundles and services offered by Onfire Health Partner Clinicians and Practitioners through the Onfire platform. Contributions may not be transferred to other individuals, converted to general cash, or applied to goods or services outside the Onfire partner network.
2.4 Refund at Termination Only
Unused Membership Account contributions are returned to the member via ACH Member Credit to the member's Linked Bank Account only upon termination of the member's Onfire Membership, net of any fees owed and pending service payments. Refunds are not available mid-term or at any other time during the membership. Onfire Health does not pay interest on any Membership Account balance.
2.5 Segregated Account
Member contributions are held by Onfire Health in a segregated bank account maintained in the name of Onfire Health members, separate from Onfire Health's own operating funds. Onfire pays Partner Clinics from this segregated account, net of applicable Onfire Health fees and expenses.
2.6 Plan Modifications
Onfire Health reserves the right to modify plan features, Subscription Limits, Membership Fees, and other plan terms upon 30 days' advance written notice to enrolled members. Material changes will not affect the current plan term for enrolled members. Updated Plan Terms will be published at onfirehealth.com/plan-terms.
2.7 Relationship to Membership Agreement
These Plan Terms are incorporated into and governed by the Onfire Health Membership Agreement. Capitalized terms used but not defined in these Plan Terms have the meanings given in the Membership Agreement. In the event of any conflict, the Membership Agreement controls.
